    We are required by federal and state law to maintain the privacy of your health information. You are a valued patient and we are vitally interested in protecting the privacy of your protected health information.

    We are also required to notify you about our privacy practices, our legal duties, and your rights concerning your health information. We must follow the privacy practices that are described in the notice while it is in effect. This notice takes effect April 14, 2003, and will remain in effect until we replace it.

    We will provide you with a Notice of Privacy Practices when you first visit our office. This Notice of Privacy Practices describes how we may use and disclose your protected health information (PHI) to carry out treatment, payment or health care operations (TPO) and for other purposes that are permitted or required by law.

    It also describes your rights to access and control your protected health information. “Protected health information” is information about you, including demographic information, that may identify you and that relates to your past, present or future physical or mental health or condition and related health care services.
