Your First Visit

Your first appointment will include a limited oral evaluation, discussion of your diagnosis and treatment options, and treatment planning. Your doctor will review your symptoms and examine and test the tooth or teeth in question, as well as the surrounding teeth and gum tissue. Diagnostic X-rays will be taken as needed to help determine the source of your symptoms and establish a diagnosis.

If additional imaging is clinically indicated, a CBCT (3D) scan may be recommended to provide a more detailed evaluation and assist with diagnosis and treatment planning.

In many cases, root canal treatment can be completed during the same visit as your evaluation. More complex cases may require additional appointments.

Before Your Appointment

To help us prepare for your visit and keep your appointment running smoothly, please complete all required patient forms before arriving for your appointment.

Please have the following information available:

• Your referral from your dentist or referring doctor and any available X-rays
• A current list of all medications you are taking
• Your insurance information and any required insurance forms
• All required New Patient forms, completed in full

If you prefer to complete your paperwork using printed forms, you may select and download the forms below. Please complete all required forms and bring them with you to your appointment.

Whether completed online or using printed forms, all required paperwork must be completed prior to your appointment. This allows our team to prepare for your visit and helps keep your time in our office focused on your care.
